NOTICE TO TERMINATING EMPLOYEES IN TEXAS

If you meet certain criteria, Texas law requires your group health plan to allow you to continue coverage for six months. The six-month "continuation period" begins after any federal COBRA extension period ends. 


To be eligible, you must have been continuously covered under your former employer's group health plan for at least three months immediately prior to your termination. If you meet these criteria, please contact our customer service department during our business hours to obtain more information.


Finally, Texas law requires your plan to communicate information on the Texas Health Insurance Risk Pool (Health Pool). This pool is available to Texas residents who can't secure medical insurance due to their pre-existing medical conditions. To learn more, please contact the Health Pool at:
