Clarity Enrollment Confirmation Letter

As the third-party administrator for your continuation of coverage benefits we are reaching out to confirm and review some important information with you. Our focus is to ensure you understand your responsibilities for continuation of coverage.

You will soon receive payment coupons or a billing statement for each calendar month. The cost for coverage is due on the 1st day of the calendar month and payment is due no later than the Last Day to Pay on the coupon. If your monthly payment is not received before the grace period end date, your coverage may be terminated.  

It is very important that you log in to the Clarity Benefit Solutions Portal to make your payment or mail your payment via USPS First Class mail before the end of the grace period. Timely receipt of the payment will be based on the online payment date or the postmark date. If your payment arrives via mail after the last day to pay and the envelope does not contain a postmark, the payment may be deemed late, and coverage may be terminated.
