Your open enrollment dates are reflected on the mailed notice you received. During the open enrollment
period, you have an opportunity to review, renew, or make changes to your current health coverage. After the open enrollment period closes, changes to your benefits may only be made if you experience a Qualifying Life Event.
You currently participate in one or more of your prior employer's group health plans through continuation of
coverage under ? CoverageType? .
Your Coverage Options During Open Enrollment
You now have two options for continuing health coverage during this open enrollment period:
Explore private health insurance options that may better meet your needs, or
Continue your ?Coverage Type? coverage through Clarity Benefit Solutions
To review both options and take action, visit: www.claritybenefitsolutions.com/COBRA
If You Choose to Continue Your ?Coverage Type? Coverage
You may make changes to your existing coverage(s) just as any active employee would. This is your
opportunity to:
Change your elected coverage
Add or remove eligible dependents from your elected plan(s)
Please ensure you complete your benefit selections by the end of the open enrollment period.
The table in your mailed notice shows your currently elected benefit(s) and plan(s).
If you do not make a selection during open enrollment, your employer may automatically:
Renew you on your existing plan,
Drop you from coverage, or Move you to a new plan.
To ensure you receive your preferred benefits, please go to the link above and select either ?Coverage Type? coverage or a private insurance option before your open enrollment deadline to review your plan options and make your selections.
Please note: The open enrollment period does not extend your continuation of coverage period.
